New Phaco Techniques – Update

Purpose: Sleevles bimanual phaco without corneal burn is possible. New separated irrigation devices have to be adopted to the U/S-phaco system. Two-port and three-port incisions have been evaluated.
U/S-phaco techniques provide high effectivity, predictable savety and continous innovations. New technologies of U/S-phaco machines help to develop new operation techniques. As laser technology, U/S-technology has potential to operate with microincisions and without heat.
Method: Different manufacturers (ALCON, AMO, Geuder, Oertli, Ruck, Staar) use different technologies to reduce temperature at the tip during emulsification: laser-like ultrashort, but high ultrasound energies, combinations of mechanical energy like rotating Tipps, water jet and energy in the sonic mode.
Discussion: Laser-phacoemulsification has been developed to demonstrate ultrasmall incisions, no heat at the Tipp and high standard of tissue protection. A draw back was the lack of energy to emulsify cataracts of +3 and more hardness. New technologies of U/S delivery help to emulsify the nucleus without heat. Thus, the cooling irrigation BSS bypass around the phaco needle is no longer neccassary. Irrigation and phaco/aspiration line can be separated. Irrigation and aspiration and also vitrectomy have used it, the bimanual way of acting. Bimanual phaco helps to use incision below 1 mm – if desired.
