Autologous Serum versus Hyaluronic Acid for Treatment of Corneal Lesions after Vitrectomy in Diabetic Patients - First Results of a Prospective Randomized Study

Schulze S. D., Sekundo W., Kroll P.
Department of Ophthalmology, Philipps University, Marburg

Purpose: Autologous serum and hyaluronic acid showed to be effective, nonexpensive and safe in treatment of epithelial lesions of the cornea. The purpose of this study was to evaluate whether one of the agents has advantages in treatment of such lesions in patients with reduced wound healing like diabetic patients.
Method: This prospective randomized ongoing study includes all diabetic patients undergoing pars plana vitrectomy (first surgery, most of them combined with phaco and IOL) with abrasion of the corneal epithelium if required for better intraoperative visibility. In this case a standardized 8 mm diameter epithelial removal is performed. In addition to the usual topical therapy one group is treated with hourly application of autologous serum, the other group with hyaluronic acid drops (Vislube®). We evaluate the time needed for closure of the corneal epithelium as well as other parameters such as patient's age, sex, duration of surgery, history of diabetes and the current HbA1c.
Results: 14 patients were enrolled (10 men, 4 women). Each group contained 7 patients. The mean age was 64.5 years, duration of diabetes 16.4 years. In average the operation took 150 minutes. The mean epithelialization time in the hyaluronic acid group was 6.7 days. The epithelium healed in the autologous serum group aft
