Eye Lid Tumor in a Patient with Histiocytosis X

Purpose: Eye lid tumors of unknown origin should receive a histology and imaging to prove the diagnosis and exclude a systemic manifestation.
Method: A Turkish patient presented with bilateral manifestation of a lipogranuloma of the eye lids and the conjunctiva. To determine the diagnosis we preformed a diagnostic excision and a computer-tomography of the skull.
Results: The histology of the tumor demonstrated cells with foamy transformed cytoplasm. Immunohistochemistry showed cells with acidophil cytoplasm, and a positive reaction with protein S-100, and positive OKTG antigens in Langerhans-cells. The CT of the skull showed a tumor of the sella that may result in a diabetes insipidus. The histology and CT demonstrated the systemic involvement of generalized Histiocytosis X.
Conclusions: A precise diagnostic of an eye lid tumor is necessary, as a systematic disease might be undetected.
