Advanced Carcinoma of the Eyelid: Early and Late Postoperative Results after Blepharoplasty

Purpose: To evaluate the postoperative results after blepharoplasty in cases with large carcinomas of the eyelid.
Method: 76 patients with advanced basal cell carcinoma (n=53), squamous cell carcinoma (n=21) and adenocarcinoma (n=2) of the eyelid were included in the study. Depending on the location and the size of the tumor different techniques have been used: in 3 cases ( with advanced carcinoma of the upper eyelid – Cutler-Beard bridge flap; in 32 patients ( with large carcinoma of the inner canthal area – glabellar transposed flap; in 12 cases ( with intermediate sized carcinoma of the lower eyelid – nasojugal transposed flap; in 21 patients ( with carcinoma of the lower eyelid and external canthal area – advancement flap and in 8 patients ( with very large tumor of the lower lid – Mustarde cheek rotation flap with or without skin graft.
Results: In the first group the division of the bridge was performed after about 6 weeks. In the late postoperative period there were neither necrosis of the lower lid margin nor retraction of the upper eyelid. The techniques used in 2., 3., 4., and provided a good reconstruction of large lower-lid or canthal defects without complications. A fold “dog-ear” occurred in 3 cases of the only.
