Analysis of Optical Parameters after Cataract Surgery with Implantation of Foldable Lenses

Kutschan A., Falkenberg B., Wiegand W.
Klinikum Nord-Heidberg, Augenabteilung, Hamburg

Purpose: Earlier studies have shown that within the first few weeks after cataract surgery significant and individually not predictable changes of refraction can occur even in patients with ‚clear cornea‘ surgery and implantation of foldable lenses. The possible reasons shall be analysed.
Method: In 50 consecutive patients we determined axial length, anterior chamber depth, corneal refraction and corneal thickness with the IOL-Master and the Orbscan II, respectively, prior to cataract surgery, on the 1. postop. day and at a follow-up control after obtaining the stable refraction. We divided the patients in 3 groups: Patients with no change of refraction between the 1. postop. day and the follow up (group A), patients with hyperopic shift of refraction (group B) and patients with myopic shift of refraction (group C).
Results: In all 3 groups we measured postoperatively no significant change of the axial length. Central corneal thickness increased by 40 µm on the 1. postoperative day and had normalized at the time of control. Anterior chamber depth decreased in all groups by 0.5 mm between the 1. postop. day and the follow-up. Central corneal refraction showed a decrease in group B, an increase in group C and no changes in group A. The standard deviation, however, was very high in all groups.
Conclusions: The postoperative change of refraction (spherical equivalent) in the f
