Angiographic Findings in Adventitial Sheathotomy for Decompression of Branch Retinal Vein Occlusion

Purpose: Branch retinal vein occlusion with macular involvement frequently causes retinal ischemia and irreversible loss of visual acuity. Adventitial sheathotomy for decompression of branch retinal vein occlusion is new therapeutic option, but indication and limits are up to date not defined. We report changes of angiographic parameters of our patients.
Method: 16 patients (female: 12/16 = , age 67,2 ± 8,1 years) were examined preoperatively, six weeks, three month, six month and one year after adventitial sheathotomy for decompression of branch retinal vein occlusion. Arteriovenous passage time (AVP) of the affected and unaffected branches at first and at maximal venous filling were measured to assess retinal hemodynamics. Changes in the foveal avascular area (FAZ) were calculated to determine foveal structural changes.
Results: The early AVP of the affected branch was increased significantly at all times when compared to the unaffected branch, but it was decreased significantly from 6,7 s preoperatively to 0,9 s at 6 month (p<0,05). The AVP at maximal venous filling did not differ preoperatively (p=0,14), but was increased at six weeks and three month when compared to the AVP of the unaffected branch (p<0,05).
