Importance of the Assessment of the Site-specific Lens Constant for the SRK-II Formula with the Example of the AcriSmart IOL

Purpose: For the calculation of IOL power for cataract surgery, IOL producers recommend the site specific optimisation of the lens constant.
Method: Within a retrospective study, we analysed data (surgical technique, refraction target, preop and postop refraction etc.) in 37 eyes of 26 patients (7 male, 19 female; age 50 - 82 Jahren), which had a cataract operation with implantation of a AcriSmart IOL (model Acri.Lyc 48S-5) by Acri.Tec Co. Berlin, Germany. Cataract surgery was performed as bimanual Erbium laser phaco (PHACOLASE, Carl Zeiss Meditec AG Jena) via two 1.7 mm incisions (phaco energy m = 35.5 ± 41.5 J; phaco time m = 93.2 ± 95.2 s). The IOL was implanted via the Acriglide injector (Acri.Tec Co. Berlin, Germany) into the capsular bag. Postoperative examinations were performed 3 days and 59 (average) days postoperatively. For the calculation of the IOL power, the SRK-II formula basing on the lens constant recommended by the IOL producer had been used.
Results: The preoperative refraction target was m = -0.95 ± 0.40 D (spherical equivalent, SE). Postoperatively, a SE of m = -1.24 ± 0.71 D was reached. Therefore, there was an average myopic deviation of -029 D. The deviation from the given lens constant is statistically significant on the 9-% level (Wilcoxon test for matched pairs, n = 25, p = 0.085). The deviation is dependent on the axial le
