Intraindividual Comparison of Higher Order Aberrations after Implantation of Aspherical and Conventional IOLs Depending on Pupil Diameter

Purpose: The implantation of an aspherical IOL with negative spherical aberration should equalize the positive spherical aberration of the cornea.
Method: In 20 eyes of 10 patients (mean age 72.89 ± 7.36) with cataract, we implanted an aspherical IOL (Tecnis, Pharmacia) in one eye, in the other eye a conventional IOL (AR40e, AMO), respectively. After 3 months we measured the total (HOA RMS) and spherical aberration (Z 4.0) as well as the 3rd and 4th order aberration at pupil diameters of 3, 3.5, 4, 5 and 6 mm with a Hartmann-Shack sensor (Zywave, Bausch&Lomb). For each pupil diameter, an intraindividual comparison of the aberrations between both eyes was done. For statistical analysis we used the Wilcoxon-matched-pairs-test.
Results: After implantation of Tecnis IOLs, total HOA RMS, Z 4.0 and 4th order aberrations were lower than after implantation of AR40e IOLs. The differences between the IOLs rose with increasing pupil diameter, but reached only significant values for total HOA RMS and 4th order RMS. For 3rd order RMS neither significant differences between the IOLs nor a dependence on pupil diameter was found.
Conclusions: With implantation of aspherical IOLs positive spherical aberrations of the cornea could be reduced compared to convention
