The Dislocation of Soemmerring's Ring after Cataract Surgery - Case Report

Purpose: The Soemmerring's ring represents a secondary cataract with circular equatorial opacity. We report the case of a patient with phacogenic uveitis and glaucoma after the dislocation of Soemmerring´s ring into the vitreous 63 years after paediatric cataract surgery.
Method: A 68 year old patient was admitted to our department with acute visual acuity decrease on his left, the only functioning eye from 0,3 to hand movements. The right eye had no light perception due to a total PVR detachment with closed funnel. The examination of anterior segment showed a significant intraocular inflammation, glaucoma and a small hypopyon. Ocular ultrasound examination detected a dense structure in the vitreous. During the following pars-plana-vitrecomy this structure has been proved as Soemmerring's ring.
Results: At the last follow-up examination, 48 months after the vitrectomy, the patient had a visual acuity of 0.2. During the whole follow-up the retina remained attached, the eye showed no signs of intraocular inflammation. The intraocular pressure was 8 mm Hg with local therapy.
Conclusions: A phacogenic uveitis occurs usually short time after cataract surgery. In our case the phacogenic uveitis followed the dislocation of Soemmerring's ring into the vitreous 63 years after the cataract surgery.
