Pupil Ovalization in Patients with Phakic IOL

Purpose: Phakic IOLs are successfully implanted in patients with high myopia. Pupile ovalization occurs in up to 30% of the patients. In this study we evaluated the degree of iris ischämia in patients with pupile ovalization after phakic IOL implantation.
Method: 12 patients with phakic IOL underwent indocyanine angiography of the iris after an average follow up of 23 months. All pupiles were round in the early post operative period.
Results: 3 patients with pupile ovalization showed a delayed perfusion time in the areas with larger pupile diameter. The other 9 cases did not show a pathologic angiography of the iris. Delayed perfusion of the iris seems to correlate with the degree of iris ovalization.
Conclusions: Pupile ovalization in patients with phakic IOLs is not due to a mechanic but an ischemic pathology and is therefore not only a cosmetic problem. Further consequences of this pathology have to be evaluated.
