First Experience with the Use of a Non-mechanical Single-use Trephine: Parameters and Ablation Pattern

Purpose: A newly developed non-mechanical Excimer laser trephine with a fiber optic unit is suspected to have distinct advantages when used for penetrating keratoplasty. Here we describe the first experience with an industrially produced simple disposable trephine (Gebauer GmbH, Neuhausen; Geuder AG, Heidelberg) which can be used in conjunction with small, mobile, laser units.
Method: A motor driven trephine was developed which can be centered by use of a suction ring. It is used in conjunction with a fiber that is connected to a small, mobile, commercially available 308nm Excimer laser (TUI Laser AG, Munich). In a pre-clinical study freshly enucleated pig and human eyes were used. Conventional histology of cuts with the laser trephine was performed and investigated to evaluate the quality of the cuts and possible mechanically induced damages.
Results: The laser trephine is easy to use and allows to precisely cut both porcine and human corneas. Histology does not showed mechanically induced damage. The quality of the cutting edges was smooth, thermal damage of less than 10µm could be demonstrated.
Conclusions: In comparison to former investigations we could demonstrate that the use of a industrially produced single use trephine increases the quality of the cut and therefore the accuracy of the trephination. Our new laser trephine system offers a
