Intravitreal Triamcinolone as Treatment of Chronic Focal Immunologic Corneal Graft Reaction

Purpose: To report about the treatment of a chronic focal immunologic corneal graft reaction.
Method: A 53-years old patient presented with chronic focal immunologic corneal graft reaction and secondary angle-closure glaucoma after she had undergone several intraocular procedures including tectonic block excision of a ciliary body nevus simulating malignant melanoma; cataract surgery with implantation of an intraocular lens; and penetrating keratoplasty with goniosynechiolysis. Under topical anesthesia, a standard penetrating filtering surgery and an intravitreal injection of 25 mg of triamcinolone acetonide were performed.
Results: At the first postoperative day, the region of the focal immunologic graft reaction was markedly less edematous, and within one week, the retrocorneal precipitates and stromal edema almost completely resolved. At the last follow-up examination one month after the injection, the corneal graft was clear without any topical or systemic ophthalmic or immunosuppressive medication taken.
Conclusions: The intravitreal injection of triamcinolone acetonide may be a therapeutic option for treatment of immunologic corneal graft reactions.
