The Nature of the 'Negatve ERG' and its Correlative in Multifocal ERG Recordings
The negative configulation in scotopic bright flash ERG indicates the lower b-wave than a-wave, which is seen in patients with functional disturbance of the second order neuron, such as some kinds of CSNB, X-linked retinoschisis, and so on. In cone visual pathway of such patients, the long-flash cone ERG is requested to evaluate the function of ON- and OFF-pathway separatedly. The optimal conditions for recording multifocal ON-OFF responses will be described. I want to emphasize why such evaluation is important, using complete CSNB which is a model disease of the selective defect of rod and cone ON- pathway. In addition, some interesting clinical cases will be introduced to demonstrate ON- or OFF-dominant macula which is shown by multifocal ERG.
