Incorrect Position of the Lid in Consideration of Ptosis

Basis of operative therapy is the classification of ptosis dependent on its pathogenesis. Insufficient diagnosis particularly in regard to the comlipcated cases of ptosis and use of tarsusreducing methods lead to complications ( lagophthalmus, keratopathy, lidedgeinstapabilitiy with: entropium, knikformaton, etc.) which partially are difficult to correct. In the presentation modern techniques in ptosisurgery in consideration of functional-anatomical measures will be shown. In the treatment of congenital ptosis with deficient function of the levator muscle it is possible to avoid the application of the one-sided frontalis-loop-suspension and its known disadvantages by adequate preparation and resection of anatomical lead. The exact and complete preparation of the aponeurosis of the levator palpebrae muscle and its refixation to the tarsus allows not only to improve the postoperative results and therefore reduces the number of reoperations but also reduces noticeably operationlength and costs. Afterwards techniques of reconstruction of the lidcrease will be discussed to draw attention from the functional also to the cosmetic-aesthetic aspect of ptosissurgery.
