360-degree Argon-endolaser Retinopexy in Primary Vitrectomy in Eyes with Rhegmatogenous Retinal Detachment

Purpose: There is an increasing evidence, that pars plana vitrectomy constitutes a reasonable alternative to buckling procedures in the treatment of rhegmatogenous retinal detachment. In order to investigate the effectiveness of pars plana vitrectomy together with 360-degree endophotocoagulation in eyes with uncomplicated rhegmatogenous macula-off retinal detachment with more than 50 % of retina detached without the use of any encircling bands.
Method: Twenty-one consecutive patients with uncomplicated rhegmatogenous retinal detachment were treated by the means of standard three-port pars plana vitrectomy. The main outcome measure was the reattachment rate of the retina and the postoperative presence of PVR.
Results: The surgical outcome in 20 eyes (95 %) remained successful during the whole follow up period. In one eye the retina redetached four weeks after initial surgery due to PVR and has been reattached in a second procedure. There was no PVR - activity detected in the remaining 20 eyes. Cataract was progressive in 5 (50 %) out of 10 phakic eyes.
Conclusions: Pars plana vitrectomy with gas or silicon oil endotamponade appears to be an effective therapeutical approach in uncomplicated rhegmatogenous macula-off retinal detachment with more then 50 % of retina detached.
