The Effects of the Retinoblastoma Therapy and the Incidence of Second Tumor

Purpose: The investigation of the relationship between the incidence of second tumors in bilateral retinoblastoma (rb) patients treated with external beam radiation (EBR) and the age of the patient at the time of treatment.
Method: Between 1960-2002 patients with rb were treated in the University Hospital Essen. 602 cases of bilateral rb and 439 cases of unilateral rb were observed with a follow up of at least 1 year.(mean: 14 years, range: 1- 44 years). The Kaplan-Meier Life Table and the Log Rank Test were used.
Results: Within the group of bilateral rb 44 (9,5%) patients developed a second non ocular malignancies (SNOM). Microscopic examination of 30 tumors inside the field of irrdiation showed 7 osteosarcoma and 20 soft tissue malignant tumors and other malignancies. Outside the field of irradiation 18 malignant tumors were recorded, microscopic examination showed 7 osteosarcoma and 1 soft tissue tumor and a variety of different other malignant tumors. Statistical analysis showed no significant relationship between the time of radiation and the incidence of soft tissue tumors and osteosarcomas inside and outside the field of irradiation. The analysis showed no relation between the age at the time of EBR and the incidence of second non ocular malignancies inside the field of irradiation(p = 0,525; Log-Rank
