Application of a Potassium-Titanyl-Phosphate (KTP)-Laser on Human Sclera, Choroidea and Choroidal Melanoma

Purpose: To evaluate the effects and side-effects of a KTP-Laser (532nm) on human sclera, choroidea and choroidal melanoma for future use in vaporisation of choroidal melanomas.
Method: 4 human cadaver eyes and one enucleated eye from a melanoma-patient were used for laser examinations with a NWL-Laserscope KTP-Laser. Application by a flexible light fibre (diameter 0,3mm). Variation of parameters: Time of application (1s-8s), ab externo (sclera), ab interno (choroidea with sclera), continuous wave (CW)-mode (2W-8W), star pulse-mode (60W/20ms, 40W/50ms, 20W/50ms), in air, in water, contact-mode and non contact-mode. We did examination with scanning electron microscopy and histological staining (Masson-Trichrome) with quantitative analysis of the zones of thermal injury.
Results: Observed zones in histology (sclera): zone of vaporisation, zone of carbonisation, zone of strong denaturation, edema and zone of no visible influence on sclera. Penetrations up to 1000µm directly proportional to applied laser power. Non contact-mode: perforation of the sclera even with low energies. Strong choroidal shrinkage in CW-mode. Pulse-mode: exact choroidal vaporisation. In water mechanical destructions of tissue, but no smaller thermal injury. Selective thermolysis of scleral blood-vessels (all modes).
